Classifichiamo abitualmente gli attacchi di panico nella categoria dei nuovi sintomi, ovvero quelle configurazioni di patologia diverse da quelle trasmesseci dalla psicoanalisi tradizionale, diverse dalle categorie provenienti dall’esperienza freudiana e dalla parte classica dell’insegnamento di Lacan. La diagnosi tipica si divideva in isteria, nevrosi ossessive e in psicosi. C’era un posto anche per le fobie, affini al fenomeno del panico, ma tra le nuove manifestazioni sintomatiche l'attacco di panico ha una sua specificità, ed è la punta emergente dei nuovi sintomi.
Per parlare di questo occorre anche vedere come le espressioni sintomatiche rispondano a determinate configurazioni sociali. E’ il fenomeno della società consumista, in cui si sviluppa il terreno di coltura per le recenti forme di patologia, e che provoca un riorientamento nel rapporto tra le persone e le cose. Di panico ha parlato invece uno psichiatra, Donald Klein il quale dando degli antidepressivi a pazienti che non erano depressi, ma presentavano solo manifestazioni d’ansia, ha riscontrato una sedazione degli stati ansiosi. La storia della farmacologia psichiatrica è fatta tutta di risultati che vengono un po’ per caso, non c'è mai una ratio di fondo che guida il progetto farmacologico. Si tratta sempre di trovate empiriche: non si sa perché le cose avvengano. Allora si inventano teorie, si suppone che alcune molecole agiscano su altre molecole, la dopamina, l’adrenalina, senza che ci siano test o analisi per misurare i livelli di dopamina o di adrenalina nel cervello.
Detto questo è tuttavia per noi interessante valorizzare la differenza tra il panico, come ci viene dalla psichiatria, e l'angoscia. Ma non si tratta di dogmatizzare il fatto che l'angoscia proviene dalla psicoanalisi e il panico dalla psichiatria. Del panico troviamo traccia negli scritti di Freud, ma non solo: ne troviamo anche le chiavi concettuali. Penso in particolare a quello che scrive in Psicologia delle masse e analisi dell'io dove descrive il fenomeno del panico conseguente allo scioglimento dei legami libidici di una massa. Fa gli esempi delle istituzioni di massa più classiche: l'esercito e la chiesa. Ciò che costituisce un legame tra gli individui che formano la massa passa attraverso la figura del capo, che incarna l'ideale con cui tutti si identificano e che la media il legame tra i componenti del gruppo.
